Full Name: Full name must be printed in HOA, and the content must be correct on the ID card and household registration book.

Date, month, year of birth: must write full date, month, year of birth and in DD/MM/YYYY format. Ex: June 2, 2001.

Permanent residence: write according to the address listed on your household registration, Capitalize the first letters of place names or personal names. For example: Commune A, District B, Province C.

When do you need to inform whom? Where?: you can write the names of father and mother and include the address.

Phone number: Enter your phone number or family phone number.

For example: if you were born on March 2, 2004, enter: 02 03 04

Ethnicity: Fill in 1 if you are of Kinh ethnicity, Fill in 0 if you are of another ethnicity.

Religion: Please specify which religion you follow.

For example: Buddhism, Christianity, Islam. If you do not follow any religion, fill in no.

Background: If a worker, fill in 1, Farmer fill in 2, Other professions fill in 3.

Eligible candidates: Write the same as on the exam application form, if you do not belong to any category, leave blank.

School symbol: Write the school code you want to enroll in soon.

Registration number: is the registration number on the admission paper for the national high school exam.

Academic results: based on results in school transcripts.

Date of joining the Youth Union of Ho Chi Minh City: Record according to the union book and in the correct format dd/mm/yyyy.

Date of joining the Communist Party of Vietnam: Record according to your Party membership book, if you do not have one, leave it blank

Reward and discipline: Fill in the information to be rewarded, if there is no information, write "no"

Gender: If male, fill in 0, if female, fill in 1

Permanent residence: Record the address as shown in the family's household registration book.

Which enrollment area do you belong to?: Please calculate the number depending on your area, for example, write 01 for area 1, 02 for area 2. Note, write a number in each box.

Field of study: The field you are applying to enter the school, you need to clearly state the name of the field and fill in the field code in the boxes next to it

Ethnicity, Religion: Record according to identification information of parents, siblings.
